Reclaiming Your Health
Completing cancer treatment is a major milestone, but many survivors continue to experience persistent symptoms, late effects, and uncertainty about how to optimize their long-term health. Ascent Remission provides a structured survivorship program that combines individualized medical assessment, survivorship care planning, evidence-based lifestyle interventions, and ongoing follow-up to help patients successfully transition from active treatment into survivorship. The program is designed to complement—not replace—the ongoing relationship between patients, their oncology team, and primary care providers.
What to Expect

Initial Survivorship Assessment
Comprehensive review of cancer diagnosis, treatment history, current symptoms, medications, surveillance recommendations, and survivorship goals.

Individualized Survivorship Care Plan
Development of a personalized survivorship care plan addressing follow-up care, management of late effects, preventive health, and evidence-based risk reduction.

Structured Follow-Up Visits
Five to six virtual visits over approximately three months focused on symptom management, recovery, long-term health optimization, and implementation of the survivorship care plan.

Care Coordination
Documentation is shared with the oncology team and primary care provider following each visit to promote continuity of care.

Transition Back to Long-Term Follow-up
At completion of the program, patients receive an updated survivorship care plan and transition back to their oncology and primary care teams for ongoing surveillance.
